NB: Annual membership fee is $60. Please pay your membership BEFORE you fill in this form, and make a note of payment date / identifier details. Write your name on the back of cheques. This application form will ask for those details. (When making BSB payments, please add your name to the identifier / reference field when you process your paymernt at your bank website or local branch - so that we will know the payment is from you!)

Making payments to the CCGA:

By BSB:
1. Pay your membership fee via BSB, either through your bank's website or your local branch.
So we can match your payment to you, please write your NAME at least, and payment info in the reference / identifier field.
BSB deposit forms contain a spot where you can write this 'identifer'
eg 'Sally Celebrant subs 2010-11'
You'll need the identifier info later for the application form.

BY PERSONAL CHEQUE OR POSTAL ORDER:
